A statewide survey is now open to gather community input on Nevada’s opioid response. We encourage community members, providers, and partners to participate and help shape efforts across the state.
We invite you to complete Nevada’s Statewide Opioid Assessment. This survey will help guide efforts to address opioid use, overdose prevention, treatment access, recovery services, and related public health strategies across the state. This survey is intended for people aged 18 and older. Responses will be used to better understand community needs and to inform opioid prevention, treatment, and outreach efforts throughout Nevada. The survey takes approximately 15-20 minutes to complete. Responses are anonymous, and all answers will be kept confidential.
Your participation is voluntary, and you may skip any question you do not wish to answer. Please complete the survey only once, either on paper or online. Do not include your name or any other personally identifying information. All responses will be combined and reported in summary form only. If you completed this survey at an agency or organization, please return it to the person who provided it to you.

Thank you for taking the time to help us strengthen Nevada’s response to opioid use and improve the health and well-being of communities statewide.
